In order to allow serialization of dynamic I/O with this patch
the /dev/chsc character device can only be accessed by one
single opener. Any subsequent open calls are rejected with EBUSY.

The disable slot implementation on s390 currently just detaches the
pci function from the partition - without informing the pci layer.
Fix this by calling pci_stop_and_remove_bus_device prior to the
operation.

Add a kernel parameter to be able to specify the number of pages to be
used as output buffer by the line-mode sclp driver and the vt220 sclp
driver. The current number of output pages is 6, if the service element
is unavailable the boot messages alone can fill up the output buffer.
If this happens the system blocks until the service element is working
again. For a large LPAR with many devices it is sensible to have the
ability to increase the output buffer size. To help to debug this
situation add a counter for the page-pool-empty situation and make it
available as a sclp driver attribute.
To avoid the system to stall until the service element works again
add another kernel parameter to allow to drop output buffers.

virt_to_phys on s390 currently uses the LRA instruction to translate
virtual to physical addresses. This creates an unnecessary overhead
and caused trouble with dma debugging code (when called with an
address pointing to a already unmapped page).
Just get rid of s390's implementation and use the one from
asm-generic/io.h .
Note: with this change virt_to_phys will no longer work on vmalloc'ed
addresses.

On Xen kernels, if ATC (address translation cache) is enabled, the
first PCIe DMA read from the adapter fails with an error. This is due to
a bug ASIC, which leads to a failure of 1860 ports to be initialised.
This patch includes the fix to disable Invalidated Tag Match Enable
capability by setting the bit 26 of CHIP_MISC_PRG to 0, by default it is
set to 1.

Pull late power management and ACPI fixes from Rafael Wysocki:
"Sorry about the timing of this, but ACPI-based docking stations with
PCI devices on them and ATA bays would be hardly usable with 3.10
without it. We've been working on these fixes for the last couple of
weeks and everyone involved appears to be reasonably comfortable with
them now.
The PM part is one fix for a cpufreq regression introduced recently
- Fix for an ACPI dock regression introduced by the recent rework of
the ACPI-based PCI hotplug code (acpiphp) that caused it to be
initialized before the ACPI dock driver, which is incorrect (ACPI
dock has to be initialized before acpiphp so that acpiphp can
register PCI devices on docking stations with it for PCI hotplug on
re-dock to work). From Jiang Liu.
- Fix for PCI resources allocation in the ACPI-based PCI hotplug code
(acpiphp) that makes it use the same PCI resources assignment rules
during runtime hotplug that are used during boot (the BIOS' choices
are now respected in both cases). This prevents PCI resource
allocation failures during hotplug from happening in some cases.

- Fix for ordering and synchronization issues during hot-removal of
PCI devices on docking stations. It makes the ACPI dock code carry
out the PCI devices removal synchronously during undock instead of
spawning a separate asynchronous work item to remove each of them
without even bothering to wait for all those work items to
complete. The hot-addition part is changed analogously.
- Fix for a regression (introduced a few releases ago) that removed
the code to register a hotplug notificaion handler for for ATA
ports/devices inadvertently which prevented ATA bays hotplug from
working. The missing code is added back with some improvements.

The WARN_ON condition check in IO completion path is wrong. IOtags
returned by the firmware is compared with driver/bfa iotag after masking
the retry count bits.

D-Port is a new port type created with the intention of running link
level diagnostic tests like loopback, traffic test. In static D-port
mode, user configures the port to D-port mode and starts the test, but
in dynamic D-port, once the Brocade switch port is configured to
D-port, it will reject the regular FLOGI from HBA with reason that it is
in D-port mode. So based on the reason code HBA port will turn itself into
D-port and start diagnostic test.

The 'ctl' field of the 'struct ata_taskfile' is not really dual purpose, i.e.
it is not intended for storing the alternate status register (which is mapped
at the same address in the legacy IDE controllers) in the qc_fill_rtf() method.
No other 'libata' driver except 'drivers/scsi/ipr.c' stores the alternate status
register's value in the 'ctl' field of 'qc->result_tf', hence this driver should
not do this as well...

Commit d21a1c83c7 (ARM: KVM: define KVM_ARM_MAX_VCPUS
unconditionally) changed the Kconfig logic for KVM_ARM_MAX_VCPUS to work around a
build error arising from the use of KVM_ARM_MAX_VCPUS when CONFIG_KVM=n. The
resulting Kconfig logic is a bit awkward and leaves a KVM_ARM_MAX_VCPUS always
defined in the kernel config file.
This change reverts the Kconfig logic back and adds a simple preprocessor
conditional in kvm_host.h to handle when CONFIG_KVM_ARM_MAX_VCPUS is undefined.

When performing a Stage-2 TLB invalidation, it is necessary to
make sure the write to the page tables is observable by all CPUs.
For this purpose, add a dsb instruction to __kvm_tlb_flush_vmid_ipa
before doing the TLB invalidation itself.

Not saving PAR is an unfortunate oversight. If the guest performs
an AT* operation and gets scheduled out before reading the result
of the translation from PAR, it could become corrupted by another
guest or the host.
Saving this register is made slightly more complicated as KVM also
uses it on the permission fault handling path, leading to an ugly
"stash and restore" sequence. Fortunately, this is already a slow
path so we don't really care. Also, Linux doesn't do any AT*
operation, so Linux guests are not impacted by this bug.

S2_PGD_SIZE defines the number of pages used by a stage-2 PGD
and is unused, except for a VM_BUG_ON check that missuses the
define.
As the check is very unlikely to ever triggered except in
circumstances where KVM is the least of our worries, just kill
both the define and the VM_BUG_ON check.

Admitedly, reading a MMIO register to load PC is very weird.
Writing PC to a MMIO register is probably even worse. But
the architecture doesn't forbid any of these, and injecting
a Prefetch Abort is the wrong thing to do anyway.
Remove this check altogether, and let the adventurous guest
wander into LaLaLand if they feel compelled to do so.

HYP PGDs are passed around as phys_addr_t, except just before calling
into the hypervisor init code, where they are cast to a rather weird
unsigned long long.
Just keep them around as phys_addr_t, which is what makes the most
sense.

Currently, kvmtool unconditionally declares that HVC should be used
to call PSCI, so the function numbers in the DT tell the guest
nothing about the function ID namespace or calling convention for
SMC.
We already assume that the guest will examine and honour the DT,
since there is no way it could possibly guess the KVM-specific PSCI
function IDs otherwise. So let's not encourage guests to violate
what's specified in the DT by using SMC to make the call.
