Firmware update fails with: status x17 add_status x56 on the final write
If multiple DMA buffers are used for the download, some firmware revs
have difficulty with signatures and crcs split across the dma buffer
boundaries. Resolve by making all writes be a single 4k page in length.

Whenever an I/O for a RAID volume fails with IOCStatus
MPI2_IOCSTATUS_SCSI_IOC_TERMINATED and SCSIStatus equal to
(MPI2_SCSI_STATE_TERMINATED | MPI2_SCSI_STATE_NO_SCSI_STATUS) then
return the I/O to SCSI midlayer with "DID_RESET" (i.e. retry the IO
infinite times) set in the host byte.
Previously, the driver was completing the I/O with "DID_SOFT_ERROR"
which causes the I/O to be quickly retried. However, firmware needed
more time and hence I/Os were failing.

lpfc cannot establish connection with targets that send PRLI in P2P
configurations.
If lpfc rejects a PRLI that is sent from a target the target will not
resend and will reject the PRLI send from the initiator.

We could allocate less memory than intended because we do:
bfad->regdata = kzalloc(len << 2, GFP_KERNEL);
The shift can overflow leading to a crash. This is debugfs code so the
impact is very small. I fixed the network version of this in March with
commit 13e2d5187f ("bna: integer overflow bug in debugfs").

This patch cleans up a lot of warnings when unloading the driver.
A current example of the stack trace starts with:

This is caused by the fact that host device hostX is deleted before the
SAS transport devices hostX/port-a:b.
This patch fixes this by reverting the order of device deletions.

When the hpsa module is unloaded using rmmod, dangling
symlinks remain under /sys/class/sas_phy. Fix this by
calling sas_phy_delete() rather than sas_phy_free (which,
according to comments, should not be called for PHYs that
have been set up successfully, anyway).

The scsi_debug driver incorrectly suggests there is an error with the
SCSI WRITE SAME command when the number_of_logical_blocks is greater
than 1. It will also suggest there is an error when NDOB
(no data-out buffer) is set and the number_of_logical_blocks is
greater than 0. Both are valid, fix.

In non-coherent DMA mode, kernel uses cache flushing operations to
maintain I/O coherency, so scsi's block queue should be aligned to the
value returned by dma_get_cache_alignment(). Otherwise, If a DMA buffer
and a kernel structure share a same cache line, and if the kernel
structure has dirty data, cache_invalidate (no writeback) will cause
data corruption.

On loosely ordered memory systems (PPC for example), the WQE elements
were being updated in memory, but not necessarily flushed before the
separate doorbell was written to hw which would cause hw to dma the
WQE element. Thus, the hardware occasionally received partially
updated WQE data.
Add the memory barrier after updating the WQE memory.

UFS device requires to perform bkops (back ground operations) periodically
but host can control (via auto-bkops parameter of device) when device can
perform bkops based on its performance requirements. In general, host
would like to enable the device's auto-bkops only when it's not doing any
regular data transfer but sometimes device may not behave properly if host
keeps the auto-bkops disabled. This change adds the capability to let the
device auto-bkops always enabled except suspend.

If the driver is built as a module, autoload won't work because the module
alias information is not filled. So user-space can't match the registered
device with the corresponding module.
Export the module alias information using the MODULE_DEVICE_TABLE() macro.
Before this patch:

This patch drops two incorrect usages of tcm_qla2xxx_free_cmd()
during TMR ABORT within tcm_qla2xxx_handle_data_work() and
tcm_qla2xxx_aborted_task(), which where attempting to dispatch
into workqueue context to do tcm_qla2xxx_complete_free() and
subsequently invoke transport_generic_free_cmd().
This is incorrect because during TMR ABORT target-core will
drop the outstanding se_cmd->cmd_kref references once it has
quiesced the se_cmd via transport_wait_for_tasks(), and in
the case of qla2xxx it should not attempt to do it's own
transport_generic_free_cmd() once the abort has occured.
As reported by Pascal, this was originally manifesting as a
BUG_ON(cmd->cmd_in_wq) in qlt_free_cmd() during TMR ABORT,
with a LIO backend that had sufficently high enough WRITE
latency to trigger a host side TMR ABORT_TASK.

Commit 109bade9c6 ("scsi: sg: use standard lists for sg_requests")
introduced an off-by-one error in sg_ioctl(), which was fixed by commit
bd46fc406b ("scsi: sg: off by one in sg_ioctl()").
Unfortunately commit 4759df905a ("scsi: sg: factor out
sg_fill_request_table()") moved that code, and reintroduced the
bug (perhaps due to a botched rebase). Fix it again.

A user may lower the max_sectors_kb setting in sysfs to accommodate
certain workloads. Previously we would always set the max I/O size to
either the block layer default or the optional preferred I/O size
reported by the device.
Keep the current heuristics for the initial setting of max_sectors_kb.
For subsequent invocations, only update the current queue limit if it
exceeds the capabilities of the hardware.

It's caused by skb_shared_info at the end of sk_buff was overwritten by
ISCSI_KEVENT_IF_ERROR when parsing nlmsg info from skb in iscsi_if_rx.
During the loop if skb->len == nlh->nlmsg_len and both are sizeof(*nlh),
ev = nlmsg_data(nlh) will acutally get skb_shinfo(SKB) instead and set a
new value to skb_shinfo(SKB)->nr_frags by ev->type.
This patch is to fix it by checking nlh->nlmsg_len properly there to
avoid over accessing sk_buff.

The value of "size" comes from the user. When we add "start + size" it
could lead to an integer overflow bug.
It means we vmalloc() a lot more memory than we had intended. I believe
that on 64 bit systems vmalloc() can succeed even if we ask it to
allocate huge 4GB buffers. So we would get memory corruption and likely
a crash when we call ha->isp_ops->write_optrom() and ->read_optrom().
Only root can trigger this bug.

When calling SG_GET_REQUEST_TABLE ioctl only a half-filled table is
returned; the remaining part will then contain stale kernel memory
information. This patch zeroes out the entire table to avoid this
issue.

When storvsc is sending I/O to Hyper-v, it may allocate a bigger buffer
descriptor for large data payload that can't fit into a pre-allocated
buffer descriptor. This bigger buffer is freed on return path.
If I/O request to Hyper-v fails due to ring buffer busy, the storvsc
allocated buffer descriptor should also be freed.

Commit 1bc0eb0446 ("scsi: sg: protect accesses to 'reserved' page
array") adds needed concurrency protection for the "reserve" buffer.
Some checks that are initially made outside the lock are replicated once
the lock is taken to ensure the checks and resulting decisions are made
using consistent state.
The check that a request with flag SG_FLAG_MMAP_IO set fits in the
reserve buffer also needs to be performed again under the lock to ensure
the reserve buffer length compared against matches the value in effect
when the request is linked to the reserve buffer. An -ENOMEM should be
returned in this case, instead of switching over to an indirect buffer
as for non-MMAP_IO requests.

Take f_mutex around mmap() processing to protect against races with the
SG_SET_RESERVED_SIZE ioctl. Ensure the reserve buffer length remains
consistent during the mapping operation, and set the "mmap called" flag
to prevent further changes to the reserved buffer size as an atomic
operation with the mapping.

Once the reserved page array is unused we can reset the 'res_in_use'
state; here we can do a lazy update without holding the mutex as we only
need to check against concurrent access, not concurrent release.

The 'reserved' page array is used as a short-cut for mapping data,
saving us to allocate pages per request. However, the 'reserved' array
is only capable of holding one request, so this patch introduces a mutex
for protect 'sg_fd' against concurrent accesses.
