d681c246bf powerpc: Always initialize input array when calling epapr_hypercall()

Several callers to epapr_hypercall() pass an uninitialized stack
allocated array for the input arguments, presumably because they
have no input arguments. However this can produce errors like
this one

Fix callers to this function to always zero-initialize the input

99bfbe579c powerpc/io: Fix the IO workarounds code to work with Radix
[ Upstream commit 43c6494fa1 ]

Back in 2006 Ben added some workarounds for a misbehaviour in the
Spider IO bridge used on early Cell machines, see commit
014da7ff47 ("[POWERPC] Cell "Spider" MMIO workarounds"). Later these
were made to be generic, ie. not tied specifically to Spider.

The code stashes a token in the high bits (59-48) of virtual addresses
used for IO (eg. returned from ioremap()). This works fine when using
the Hash MMU, but when we're using the Radix MMU the bits used for the
token overlap with some of the bits of the virtual address.

This is because the maximum virtual address is larger with Radix, up
to c00fffffffffffff, and in fact we use that high part of the address
range for ioremap(), see RADIX_KERN_IO_START.

As it happens the bits that are used overlap with the bits that
differentiate an IO address vs a linear map address. If the resulting
address lies outside the linear mapping we will crash (see below), if
not we just corrupt memory.

This hasn't been a problem because CONFIG_PPC_IO_WORKAROUNDS which
enables this code is usually not enabled. It is only enabled when it's
selected by PPC_CELL_NATIVE which is only selected by
PPC_IBM_CELL_BLADE and that in turn depends on BIG_ENDIAN. So in order
to hit the bug you need to build a big endian kernel, with IBM Cell
Blade support enabled, as well as Radix MMU support, and then boot
that on Power9 using Radix MMU.

Still we can fix the bug, so let's do that. We simply use fewer bits
for the token, taking the union of the restrictions on the address
from both Hash and Radix, we end up with 8 bits we can use for the
token. The only user of the token is iowa_mem_find_bus() which only
supports 8 token values, so 8 bits is plenty for that.

1ae3174fb8 powerpc/fadump: handle crash memory ranges array index overflow

Crash memory ranges is an array of memory ranges of the crashing kernel
to be exported as a dump via /proc/vmcore file. The size of the array
is set based on INIT_MEMBLOCK_REGIONS, which works alright in most cases
where memblock memory regions count is less than INIT_MEMBLOCK_REGIONS
value. But this count can grow beyond INIT_MEMBLOCK_REGIONS value since
commit 142b45a72e ("memblock: Add array resizing support").

On large memory systems with a few DLPAR operations, the memblock memory
regions count could be larger than INIT_MEMBLOCK_REGIONS value. On such
systems, registering fadump results in crash or other system failures
like below:

as array index overflow is not checked for while setting up crash memory
ranges causing memory corruption. To resolve this issue, dynamically
allocate memory for crash memory ranges and resize it incrementally,
in units of pagesize, on hitting array size limit.

e9b911a97b powerpc/64s: Add support for a store forwarding barrier at kernel entry/exit
commit a048a07d7f upstream.

On some CPUs we can prevent a vulnerability related to store-to-load
forwarding by preventing store forwarding between privilege domains,
by inserting a barrier in kernel entry and exit paths.

This is known to be the case on at least Power7, Power8 and Power9
powerpc CPUs.

Barriers must be inserted generally before the first load after moving
to a higher privilege, and after the last store before moving to a
lower privilege, HV and PR privilege transitions must be protected.

Barriers are added as patch sections, with all kernel/hypervisor entry
points patched, and the exit points to lower privilge levels patched
similarly to the RFI flush patching.

Firmware advertisement is not implemented yet, so CPU flush types
are hard coded.

62dfddfaf1 powerpc: Add security feature flags for Spectre/Meltdown

This commit adds security feature flags to reflect the settings we
receive from firmware regarding Spectre/Meltdown mitigations.

The feature names reflect the names we are given by firmware on bare
metal machines. See the hostboot source for details.

Arguably these could be firmware features, but that then requires them
to be read early in boot so they're available prior to asm feature
patching, but we don't actually want to use them for patching. We may
also want to dynamically update them in future, which would be
incompatible with the way firmware features work (at the moment at
least). So for now just make them separate flags.

5e9ea71d85 powerpc/rfi-flush: Make it possible to call setup_rfi_flush() again

For PowerVM migration we want to be able to call setup_rfi_flush()
again after we've migrated the partition.

To support that we need to check that we're not trying to allocate the
fallback flush area after memblock has gone away (i.e., boot-time only).

81da9f87ad futex: Remove duplicated code and fix undefined behaviour
commit 30d6e0a419 upstream.

There is code duplicated over all architecture's headers for
futex_atomic_op_inuser. Namely op decoding, access_ok check for uaddr,
and comparison of the result.

Remove this duplication and leave up to the arches only the needed
assembly which is now in arch_futex_atomic_op_inuser.

This effectively distributes the Will Deacon's arm64 fix for undefined
behaviour reported by UBSAN to all architectures. The fix was done in
commit 5f16a046f8 (arm64: futex: Fix undefined behaviour with
FUTEX_OP_OPARG_SHIFT usage). Look there for an example dump.

And as suggested by Thomas, check for negative oparg too, because it was
also reported to cause undefined behaviour report.

Note that s390 removed access_ok check in d12a29703 ("s390/uaccess:
remove pointless access_ok() checks") as access_ok there returns true.
We introduce it back to the helper for the sake of simplicity (it gets
optimized away anyway).

8923141371 powerpc/powernv: define a standard delay for OPAL_BUSY type retry loops
commit 34dd25de9f upstream.

This is the start of an effort to tidy up and standardise all the
delays. Existing loops have a range of delay/sleep periods from 1ms
to 20ms, and some have no delay. They all loop forever except rtc,
which times out after 10 retries, and that uses 10ms delays. So use
10ms as our standard delay. The OPAL maintainer agrees 10ms is a
reasonable starting point.

The idea is to use the same recipe everywhere, once this is proven to
work then it will be documented as an OPAL API standard. Then both
firmware and OS can agree, and if a particular call needs something
else, then that can be documented with reasoning.

This is not the end-all of this effort, it's just a relatively easy
change that fixes some existing high latency delays. There should be
provision for standardising timeouts and/or interruptible loops where
possible, so non-fatal firmware errors don't cause hangs.

59f404e2f2 powerpc/64: Fix smp_wmb barrier definition use use lwsync consistently
commit 0bfdf59890 upstream.

asm/barrier.h is not always included after asm/synch.h, which meant
it was missing __SUBARCH_HAS_LWSYNC, so in some files smp_wmb() would
be eieio when it should be lwsync. kernel/time/hrtimer.c is one case.

__SUBARCH_HAS_LWSYNC is only used in one place, so just fold it in
to where it's used. Previously with my small simulator config, 377
instances of eieio in the tree. After this patch there are 55.

e88f1e3a8b powerpc/mm: Fix virt_addr_valid() etc. on 64-bit hash

virt_addr_valid() is supposed to tell you if it's OK to call virt_to_page() on
an address. What this means in practice is that it should only return true for
addresses in the linear mapping which are backed by a valid PFN.

We are failing to properly check that the address is in the linear mapping,
because virt_to_pfn() will return a valid looking PFN for more or less any
address. That bug is actually caused by __pa(), used in virt_to_pfn().

eg: __pa(0xc000000000010000) = 0x10000  # Good
    __pa(0xd000000000010000) = 0x10000  # Bad!
    __pa(0x0000000000010000) = 0x10000  # Bad!

This started happening after commit bdbc29c19b ("powerpc: Work around gcc
miscompilation of __pa() on 64-bit") (Aug 2013), where we changed the definition
of __pa() to work around a GCC bug. Prior to that we subtracted PAGE_OFFSET from
the value passed to __pa(), meaning __pa() of a 0xd or 0x0 address would give
you something bogus back.

Until we can verify if that GCC bug is no longer an issue, or come up with
another solution, this commit does the minimal fix to make virt_addr_valid()
work, by explicitly checking that the address is in the linear mapping region.

6bdfab8084 powerpc/modules: If mprofile-kernel is enabled add it to vermagic

On powerpc we can build the kernel with two different ABIs for mcount(), which
is used by ftrace. Kernels built with one ABI do not know how to load modules
built with the other ABI. The new style ABI is called "mprofile-kernel", for
want of a better name.

Currently if we build a module using the old style ABI, and the kernel with
mprofile-kernel, when we load the module we'll oops something like:

Power9 DD1 does not implement SAO. Although it's not widely used, its presence

ec0084d082 powerpc/64s: Improve RFI L1-D cache flush fallback
commit bdcb1aefc5 upstream.

The fallback RFI flush is used when firmware does not provide a way
to flush the cache. It's a "displacement flush" that evicts useful
data by displacing it with an uninteresting buffer.

The flush has to take care to work with implementation specific cache
replacment policies, so the recipe has been in flux. The initial
slow but conservative approach is to touch all lines of a congruence
class, with dependencies between each load. It has since been
determined that a linear pattern of loads without dependencies is
sufficient, and is significantly faster.

Measuring the speed of a null syscall with RFI fallback flush enabled
gives the relative improvement:

P8 - 1.83x
P9 - 1.75x

The flush also becomes simpler and more adaptable to different cache
geometries.

efe8bc07c4 powerpc/64s: Simple RFI macro conversions
commit 222f20f140 upstream.

This commit does simple conversions of rfi/rfid to the new macros that
include the expected destination context. By simple we mean cases
where there is a single well known destination context, and it's
simply a matter of substituting the instruction for the appropriate
macro.

c3b82ebee6 powerpc/64s: Add support for RFI flush of L1-D cache

On some CPUs we can prevent the Meltdown vulnerability by flushing the
L1-D cache on exit from kernel to user mode, and from hypervisor to
guest.

This is known to be the case on at least Power7, Power8 and Power9. At
this time we do not know the status of the vulnerability on other CPUs
such as the 970 (Apple G5), pasemi CPUs (AmigaOne X1000) or Freescale
CPUs. As more information comes to light we can enable this, or other
mechanisms on those CPUs.

The vulnerability occurs when the load of an architecturally
inaccessible memory region (eg. userspace load of kernel memory) is
speculatively executed to the point where its result can influence the
address of a subsequent speculatively executed load.

In order for that to happen, the first load must hit in the L1,
because before the load is sent to the L2 the permission check is
performed. Therefore if no kernel addresses hit in the L1 the
vulnerability can not occur. We can ensure that is the case by
flushing the L1 whenever we return to userspace. Similarly for
hypervisor vs guest.

In order to flush the L1-D cache on exit, we add a section of nops at
each (h)rfi location that returns to a lower privileged context, and
patch that with some sequence. Newer firmwares are able to advertise
to us that there is a special nop instruction that flushes the L1-D.
If we do not see that advertised, we fall back to doing a displacement
flush in software.

For guest kernels we support migration between some CPU versions, and
different CPUs may use different flush instructions. So that we are
prepared to migrate to a machine with a different flush instruction
activated, we may have to patch more than one flush instruction at
boot if the hypervisor tells us to.

In the end this patch is mostly the work of Nicholas Piggin and
Michael Ellerman. However a cast of thousands contributed to analysis
of the issue, earlier versions of the patch, back ports testing etc.

8fd3f98d0f powerpc/64: Add macros for annotating the destination of rfid/hrfid

The rfid/hrfid ((Hypervisor) Return From Interrupt) instruction is
used for switching from the kernel to userspace, and from the
hypervisor to the guest kernel. However it can and is also used for
other transitions, eg. from real mode kernel code to virtual mode
kernel code, and it's not always clear from the code what the
destination context is.

To make it clearer when reading the code, add macros which encode the
expected destination context.

eae3f3ab7f powerpc/64: Fix checksum folding in csum_tcpudp_nofold and ip_fast_csum_nofold

These functions compute an IP checksum by computing a 64-bit sum and
folding it to 32 bits (the "nofold" in their names refers to folding
down to 16 bits).  However, doing (u32) (s + (s >> 32)) is not
sufficient to fold a 64-bit sum to 32 bits correctly.  The addition
can produce a carry out from bit 31, which needs to be added in to
the sum to produce the correct result.

To fix this, we copy the from64to32() function from lib/checksum.c
and use that.

Memory hotplug is leading to hash page table calls, even on radix:

  arch_add_memory
    create_section_mapping
      htab_bolt_mapping
        BUG_ON(!ppc_md.hpte_insert);

To fix, refactor {create,remove}_section_mapping() into hash__ and
radix__ variants. Leave the radix versions stubbed for now.

commit 1a92a80ad3 upstream.

There is no guarantee that the various isync's involved with
the context switch will order the update of the CPU mask with
the first TLB entry for the new context being loaded by the HW.

Be safe here and add a memory barrier to order any subsequent
load/store which may bring entries into the TLB.

The corresponding barrier on the other side already exists as
pte updates use pte_xchg() which uses __cmpxchg_u64 which has
a sync after the atomic operation.

It seems the right thing to do is to enlarge the vmalloc space on ppc
but this is not the case in the upstream kernel yet AFAIK. It is also
questionable whether that is a stable material but I will decision on
you here.

We have reverted this patch from our 4.4 based kernel.

Newer kernels do not have enlarged vmalloc space yet AFAIK so they won't
work properly eiter. This bug is quite rare though because you need a
specific HW configuration to trigger the issue - namely NUMA nodes have
to be far away from each other in the physical memory space.
