Update FC-NVME definitions to match FC-NVME r1.14 (16-020vB) plus change voted in by 2/22 FC-NVME Adhoc (see HOSTID below). These guides can be rendered in a number of formats, like HTML and PDF. In order to build the documentation, use ``make htmldocs`` or ``make pdfdocs``.
